At what point should you stop “working on extension” and start suspecting a structural problem?

You are conducting the rehabilitation process correctly, and the patient after ACL reconstruction does not regain full extension?

– Is this still a functional issue?
– Could this be Cyclops lesion?

Studies show that extension limitation can occur even in 5–25% of patients after ACL reconstruction, and one of the possible reasons for this situation is the so-called Cyclops lesion („oko Cyklopa”) – a fibrous lesion in the intercondylar fossa that may mechanically block knee extension.

Cyclops-type changes are observed in imaging studies even in 10–25% of patients after ACL reconstruction, while the symptomatic form – causing extension blockage – affects 10% of patients undergoing surgery.

If you work with patients after ACL reconstruction – as physiotherapist, doctor, movement therapist – there is a certainty that you will encounter this problem in your practice
